Requests should be submitted at least 2 weeks in advance. Please note there are two separate submission forms below; one is for Setup Requests and one is for Cleanup Requests.
You can reach our Congregational Administrator (firstname.lastname@example.org) directly with any questions. The Facilities Coordinator will follow-up with questions or concerns.

Requester: Please check each room requiring cleanup and all special cleanup anticipated. Asterisked* items should be taken care of by event team, with minimal follow-up by Facilities Coordinator. For your reference, you can find the pdf of this form here.
